Company from any and all lio bill ty due to or arising Ptnm Catawba Cextnty causing the electrical gcm'cc to the above•mcmioncd premises
being d*onnected. Temporary service is issued solely as a courtesy for the cmnpletion of construction and the building is not ro be
oeroupied until n eerli£entc of oerupanay is issued. Any violation of the above will automatically void temporary service.
The uAc of the system before final inspection is anticipated to be days or Ices, If utility cormaction rcmaims in use beyond
30 days GC will be bilied monthly per County Fee Schedule, The temporary use of the system is necessary to operate the 14vAC and to
provide lighting for die completion of the finish work. Only those ctrculrs char are necessary and/or have been inspected and appmvcd by
the electrician and electrical inspector will be energized Any 125v single -phaao circuits temporarily energized for construction purposes
will be ground fault intenupt,proteected, with the exception of lighting circuits in merlif conduit. The building will be constantly supervised
or will be securely locked when it is not supervised. The electrician, UVAC, and Plumbing contracbxs are ready 8n the xystcros to be
two. i7ad as attested below. ,
The responsible anent for the purpose of signing this form maybe the general contmetor or Owner. The GC ;A responsible
for Administrative roes unless otherwise arranged and agrees to set rap a himne account with Catawba County FSnnnce
Department for renewol of agreement, irnceded. The electrical contractor shalt coordinate this inspection with building service
