Subterranean Termites
This is to certify that at the request of your builder, Terminix Service, Inc. has installed (cr will install upon completion of final grade
and landscaping) a subterranean termite control monitoring and baiting system r: the following address:

This does not mean the property is "termite proof" It is not possible to construct any building containing cellulosic materials, or in
which cellulosic materials are stored, in a manner that will keep tetuntes out indefinitely. It does mean your builder bas conuanted to
have the monitoring and baiting system installed for the protection of this property from damage by subterranean termites.
This termite control system represents an integrated pest management approach for the protection of structures Etrm subterranean,
termites, and is intended to form the basis of an on -going program which invokes monitoring for tye presence of termite activity is and
around the structure, baiting when the presence of subterranean termites has been detected, and reswuption of monitoring for the
presence of termite activity after control bas been acbieved.
In order to give you waximun, protection against termites your builder has also prepaid a Terminix Protection Contract on
this property for a period of one year. This contract provides for periodic monitoring of the system, reinspmtions, any
retreating necessary, phis cost of repairs to the struchue or its contents (including carpeting, etc.) should termites attack
your property during the contract period. At the end of the first prepaid year this contract may be renewed by you for an
initial cost of I per year. Please call us to obtain a copy of the actual contract of complete and return the "Request for
Contract" below.

thereafter upon payment of an annual renewal. Please issue and send to me a Termite Protection Contract, in my name, and
bill me for the initial renewal on the anniversary date. If I am not frilly satisfied with the terms of the Termite Protection
Contract, I will retum it to you within thirty (30) days after receipt.
