Per information you provided, I understand there are two members of concern.
Joist D1
• Single 11 -718" BCI® 5000s 1.8 SP joist spanning 03 -09 -01 with a
01 -05 -00 cantilever over the left side of Bearing 91.
There is a 5" wide x 5" high hole in the web at 7" from the right end (02).
3 /: OSB material has been installed on both sides of the joist around the
hole as shown on attached Detail W.
Joist D2
• Single 11.7/8" BCIt9 5000s 1.8 SP joist spanning 03 -09 -01 with a
01 -05 -00 cantilever over the left side of Bearing B1.
• There is an 8" wide x 5" high hole in the web at 7" from the right end (82).
• s/V OSB material has been installed on both sides of the joist around the
hole as shown on attached Detail 'A'.

According to the design loads provided, the holes are acceptable without
additional reinforcement due to low design stresses.
