Attached are copies of the letter and component label that you require for the use of the
Mendota Vera! Heat Kit on a Mendota model DXV35 gas fireplace. The label will need to
be installed by an Intertek inspector. He will be contacting you shortly to schedule a time
to go to the site.

Intertek Testing Services NA, Inc. (Intertek) has evaluated the use of the Johnson
Gas Appliance Co. Mendota Versi Heat Kit #AA-1 1-00390 on models OXV60 and
DXV42. Based on the results of these tests, the Versi Heat Kit is acceptable for
use on the following Mendota models when installed per the instructions included
with the kit:
