Construction Section survey of your facility, As a result of the survey conducted on March 22, 2006, no
deficiencies were noted at this tune.; We have forwarded IFS - Construction Section's recommendation of
approval for two ambulatory clients to Mental Health Licetnsure and Certification Section:.
e Licensee needs to notify the Division of Facility Services - Mental Health Licensure and Certification
Section in writing when changes occur in the building physical plant, client ambulation status or when the total
capacity of the homeincreases.
Please check with your local building official prior to making changes to your facility to verify if you need to
