BASIC

lines. The certificatWi will cov r the evaluation of the industrial equi nt is i h in the
compliance f NFPA y G , NFPA 79 (Electrical Standards for Industrial chine
and UL 508A (Standard for ndustrial Control Panels). A written letter, signed and
sealed, will be providedfor o 3ch piece of equipment for 'your records. The letter will
summarize the type of equipment as well as it, compliance `th the above listed
standards. We shall not ms � e any visit prig to all required con rt r from 50HZ t
have been in t Iled &:ng with any required disconnects, Those devices shall
have a third party listing if quired by the NCS13C. We need one to two weeks notice
before we can makeour visit If any changes are required to the equipment and
subsequent visits are requirg� J, those shall be invoiced d n an hourly basis,
Compensation for the above tlln d Basic Services shalt be on a fixed fee basis as
follows,

Additional site visits shall t o compensated for on an hourly basis plus expenses where
the time and mileage sty ,:no finish from our office, The rate is as follows:
