Corrected drawings are enclosed for re- submittal, and the following is my
response to Plan Reviews as stated below.
Fire Prevention Bureau Review of December 2 1010: (first review)
Item #I Drawing Sheet EFP is void. Refer to enclosed Drawing Sheet C -I dated

Item #2 Refer to enclosed Drawing Sheet A -2 -1 dated February 23, 2011 for
construction that existed before work without a permit was performed.
Item #3 Refer to Drawing Sheet A -3.1 for Interior Wall and Ceiling Finish
Requirements by Occupancy. This is a Class C smoke and flame spread.
Note: On Drawing Sheet A -3.1, wallpaper and sheet vinyl shall be removed and Class C i
materials installed as per Note on this Drawing.
Also, see attached Meeting Notes of February 9, 2011.

1. Architect requested church write a letter to Jeff Hinkle stating the following:
a. No exterior windows installed.
b. No mechanical work has been done.
c. No electrical work has been done.
d. Jeff Hinkle agreed with writing this letter.
2. Kelly Davis stated that the existing fire alarm system in the multi - purpose
fellowship hall is not an issue.
3. Dallas Abee stated that this portion of the building is Type III construction.

4. The platform and walls were installed without a permit.
5. Jeff Hinkle stated that the screen walls rating, flame spread and smoke
characteristics are not issues. However, the sheet vinyl is an issue.
6. Jeff Hinkle also stated that the door to exit to the outside stair may be removed.

Hickory, Inc. has not had any additional renovations done to our facilities
here at 1301 12 Street Drive since our last major renovation project
completed in 2005. All work has been done with proper permit. There were
questions of windows replaced, mechanical additions, new HVAC
installations & electrical work done without permit. As a walk -thru was
performed that same day, it was clearly understood that no additional work
had been performed without your previous approval with permit.
Thank you for coming out to clear up this matter. We are happy to comply
with the authorities in all matters pertaining to safety & proper building
codes.
