Dear Madam/Sir:
On October 01, 2010, in accordance with N.C. General Statute 130A- 248(d), your facility was billed for the $75.00 Food and Lodging
annual fee for the fiscal year which ends June 30, 2011. When payment was not received within 45 days, a second invoice, which
included a $25.00 late payment fee notice, was mailed November 15, 2010. The additional 15 days allowed by the law to pay the
initial and the late fees have now elapsed.
A notice of intent to suspend your permit for this facility was mailed on December 06, 2010. This notice of intent to suspend notified
you that if no payment was received or if a petition for a contested case hearing was not filed within 30 days, the permit would be
suspended. This office has received no payment, and no petition for a contested case hearing has been filed to date. The 30 days have
now elapsed.
This letter is to inform you that the permit to operate your facility is hereby suspended immediately. Be advised that violation
of GS 130A -248, which prohibits the operation of any food and lodging facility without a valid permit, will result in appropriate legal
action being initiated.
If you pay the $100.00 fee plus $150.00 reinstatement fee, the permit to operate this facility will be reinstated. However, the
suspension of your permit remains in effect until you receive written notice from this office that the suspension has been lifted.
Payment may be made by the submission of a check made payable to "DENR" in the amount of $250.00 to: Inspections, Statistics,
and Fees Program, N.C. Department of Environment and Natural Resources, 1632 Mail Service Center, Raleigh, NC 27699-
1632.
