System Installation Comments: Permit for a supply line network(easement)from lot 72-75 wynswept to the
corresponding off site lots 72a-75a. supply lines originally installed by a utilities installer
(not a septic installer)at some unknown date without EH knowledge or inspection.
Holes dug over supply lines to examine pipes on 11/28/22. pipes used were not
acceptable per pictures bent to project engineer and lines appeared to be installed
outside of easement in places. supply lines installed outside of easement verified by
surveyor on 12/9/22. as built pipe location received on 12/19 and acceptable pipes used
for supply lines, but pressure release valves not installed. Letter from engineer on 1/20/
23 saying lines passed pressure test.

1.All maintenance, monitoring, &performance requirements shall be in accordance with 15A NCAC 18.1900, Rule.1961
