System Installation Comments: *Septic system existing. See state permit#10496 issued 7/20/84.Original facility on
site was a meat processing plant. Wastewater was industrial process and septic
system was classified as Type V. Meat processing building has been converted to
storage(no plumbing). New home was connected to existing septic system which will
now be classified as Type Ila.
*Site had an existing grease trap, 2 existing septic tanks and 2 gravel beds.While
connecting the new home to the existing septic system, one septic tank was
structurally compromised by knocking out a new inlet hole near the tank mid-seam.This
existing septic tank was abandoned. The other existing septic tank was properly
connected to the new home and one of the gravel beds.The second gravel bed was left
in tact, undisturbed and is not currently hooked up.The existing grease trap was left in
tact and is not currently hooked up.

1.All maintenance, monitoring, &performance requirements shall be in accordance with 15A NCAC 18.1900, Rule.1961
