Site Address: 7039 WILSON RD, SHERRILLS FORD NC 28673 Name on Permit: DAVID BARRINGER Property Size: Acres 0.54 Directions: Right off of Mt Pleasant Rd onto Locke Dr. Take right onto Wilson Rd. 2nd lot on left. *SEPTIC AREA MUST BE FENCED OFF TO PROTECT THE AREA, SINCE ALL AVAILABLE SPACE IS USED FOR SYSTEM AND REPAIR. *Before an operation permit can be issued a signed maintenance agreement will have to be submitted to Catawba County Environmental Health between the owner and the ORC(operator in responsible charge). *Do not cut, drive,fill, or grade over septic or repair areas. *Septic system must be 10 ft from side property lines, 10 ft from front water line, 100 ft from wells, 5 ft from building foundations and appurtenances, and out of right-of-ways and easements. *Install 1,500 gallon septic tank(risers required if top of tank is greater than 6 inches below land surface)and a 1,500 gallon pump tank with risers. Floats must be attached to a float tree. *Pump tank will pump to a LPP valve box with 5 gate valves that will feed 5 septic lines. *Install 320 linear feet of 50% reduction system(vertical), proposed 5 lines total on contour. *Lines 1, 3,and 5 laid out in the field with inferred lines for lines 2 and 4. *1st line(flagged white)is 52 ft(12 panels), 2nd line(inferred)is 52 ft(12 panels), 3rd line(flagged green) is 78 ft (18 panels). 4th line(inferred) is 69 ft(16 panels). 5th line(flagged red)is 69 ft(16 panels). Total number of PPBPS panels is 74. *This design utilizes 3/16 holes in the LPP lines with 2 ft of pressure head. *Rake and lime the trench side walls. *Seal both the outer and inner compartments of the PPBPS with foam. The outer seal is a complete seal and the inner seal is only up to the connecting pipe. *Backfill sand must be of ASTM C-33 specifications or approved by T&J Panel. *Final grade of septic area must shed surface water off and away from system. *All gutter drains must be routed away from the septic system. *well should be 5 ft from property lines; 25 ft from structures; 100 ft from septic systems; and out of right-of-way s and easements. The issuance of this permit by the Health Department does not guarantee the issuance of other permits. It is the responsibility of the applicant /property owner to insure that all Catawba County Planning/Zoning and Building Inspections requirements are met. This Authorization to Construct Permit is subject to revocation if the site plan,plat or the intended use changes,or if site conditions are altered. The Authorization to Construct Permit is not affected by a change in ownership of the property. This permit was issued in compliance with the provisions of the North Carolina'Laws and Rules for Sewage Treatment and Disposal Systems'(15A NCAC 18A.1900).
