The Environmental Health Division of Catawba County Public Health intends to revoke your
Improvement Permit and Authorization to Construct 30 days from the date of this notice.
If the permits are revoked,you must apply for a new Improvement Permit and Authorization to
Construct(IP/AC)and meet the requirements of the current laws and rules necessary to obtain a new
IP/AC.
You have a right to an informal review of this decision.You may request an informal review by the
environmental health supervisor at the local health department.You may also request an informal
review by the NC Department of Health and Human Services Regional Soil Scientist.A request for
informal review must be made in writing to the local health department.
You also have a right to a formal appeal of this decision. To pursue a formal appeal,you must file a
petition for a contested case hearing with the Office of Administrative Hearings 1711 New Hope Rd,
Raleigh, NC 27609. You may write the Office of Administrative Hearings,call the office at(984) 236-
1850 or get a copy of the petition form from the OAH web site at http://www.oah.nc.gov. The petition

Catawba County Environmental Health has not evaluated soils or site conditions for this property. Furthermore,
CCEH has not field-verified soil descriptions submitted by the Licensed Soil Scientist. CCEH exclusively used
the LSS evaluation and system design proposal to permit the septic system and future septic repair area.
Catawba County Environmental Health is not responsible or liable for accuracy of soil descriptions or septic
system design for this permit(s).
Prior to CCEH's inspection of the system installation, all property corners must be surveyed, all property lines
must be marked, and the 50 ft. lake setback, as applicable, must be surveyed. No deviation from the permit is
permissible. If the system cannot be installed exactly as proposed by the LSS, maintaining all applicable
setbacks under 15A NCAC 18A.1900, the property owner is responsible for a LSS resubmittal under SL 2018-1
14 Section 11.(c); new permit fees applicable. Do not drive, grade, cut, or fill over the septic system area/septic
repair area. Damage to the septic areas will result in permit revocation.
**PLEASE NOTIFY Catawba County EHS of the scheduled septic installation date, at least one week in
advance, so a pre-installation site visit can be made. Call 828-851-6547.
*The septic drain lines must be flagged in by the system designer(LSS), before the septic installation can begin
*The water line must be at least 10 feet from any part of the septic system.
*Weil Driller must contact EHS 1 week prior to well installation at 828-851-6547 to verify setbacks.
*Well must be minimum: 50'from any part of septic system, 25'from building foundations, 15'from overhead

/property owner to insure that all Catawba County Planning/Zoning and Building Inspections requirements are met. This Authorization to
Construct Permit is subject to revocation if the site plan,plat or the intended use changes,or if site conditions are altered. The Authorization to
Construct Permit is not affected by a change in ownership of the property. This permit was issued in compliance with the provisions of the
North Carolina'Laws and Rules for Sewage Treatment and Disposal Systems' (I5A NCAC I 8A.1900). Neither Catawba County nor the

This Licensed Soil Scientist Evaluation is bein2 submitted pursuant to and meets the requirements ofSL 2018-114 Section'11.(c).
A soil/site evaluation and wastewater system layout/design was performed on the above referenced 0.94 acre parcel to serve a 2 bedroom
accessory dwelling+a 2-bedroom residence(maximum 8 occupants)with a total peak wastewater flow of 480 gallons per day. Work
was conducted in accordance with the NC .1900 Rules for Sewage Treatment and Disposal Systems.
Soil was evaluated on this property with a hand auger. Detailed soil horizon descriptions are included in this report with corresponding
boring locations shown on the attached maps of the property. All borings were located using a Trimble Geo7X GPS unit. Existing
wells were not observed within 50+feet of the approved septic system area.
This report requests permitting installation of a gravity fed 25%reduction trench wastewater system with 440 linear feet of nitrification
line to serve a 2 bedroom accessory dwelling+a 2 bedroom main residence per the attached design criteria and conditions spelled out
in this report. Repair for this site is designated as PPBPS(Vertical)trench at 0.275 LTAR.
This report is a total of 4 pages and must not be used or interpreted without the entire report available.
