The AOWE/PE submittal shall be accompanied by a signed and dated statement from the applicant (owner or owner's
legal representative) that reads as follows:
"The plans or evaluations attached to this application are to be used to issue an Authorization to Construct in
accordance with G.S. 130A-335(a2), (a5) and (a6)." t
