On behalf of Joseph and Elizabeth O'Rourke, I am requesting an Authorization to Operate for the septic
system installed at the above referenced address. The following documentation is attached, in
accordance with the Engineered Option Permit, Part 3 requirements.

The septic system installed on my referenced property was engineered by Brian N. Groves, PE and
installed by Lago Vista LLC.The below signed recognizes the following items:
1. I accept the system installed on my property.
2. I understand that periodic pumping of the septic tank will be required to prevent over-
accumulation of solids. Disposing of grease, kitchen waste (i.e.garbage disposal),flushabte
wipes or other hygiene products is detrimental to the system and will require increased
maintenance and pumping needs.

Per Robbie's email, I think if you reply to this email stating that you are ok with the closeout meeting not being
held with the LSS and homeowner present, then we should be good to go. Otherwise, if you wish to decline
accepting the meeting that was held last week, we can wait until Larry is back in town(the 18th) and Carlos
gets back from Mexico (TBD).
It is not my intention to pressure your decision one way or the other-the nature of people's travel schedules in
the summer time has made it difficult to get all parties present at the same time.

Hope your vacation is going well. I was able to meet with the County (Robbie Phelps)and Carlos to closeout
your septic system today. Everything went smoothly-the only item we noted was that the well was placed a
few feet away from the original location, so I am going to update the As-Built drawing set and resubmit to
Robbie shortly.

While we were onsite, Robbie pointed out that the rules specify that I must have it in writing that you approve
of having the onsite closeout meeting without you being present. If you do not consent, then Robbie said he is
happy to meet you onsite to review it, and I can also make it a point to be there for the meeting. We discussed
the challenges of having this meeting in the summer(you're on vacation at the moment, Carlos leaving for
Mexico for 2 weeks tomorrow, Larry in Minneapolis until the 18th) and the need to go ahead and close out
the project even with several people absent so that we can keep your project on schedule.
Please reply all if this is ok with you, or if you want to hold an additional closeout meeting once you are back.

Gentlemen,
According to G.S. 130A-336.1(j)the LSS and the owner are required to be at the post construction conference. See below:
(j)Post-Construction Conference.-The professional engineer designing the wastewater system shall hold a post-construction
conference with the owner of the wastewater system;the licensed soil scientist or licensed geologist who performed the soils
evaluation for the wastewater system;the on-site wastewater system contractor, certified pursuant to Article 5 of Chapter 90A of
the General Statutes,who installed the wastewater system;the certified operator of the wastewater system, if any;and
representatives from the local health department and,as applicable,the Department.The post-construction conference shall
include start-up of the wastewater system and any required verification of system design or system components.The post-
construction conference required by this subsection may be waived for Type I, II, and Ill wastewater systems,as listed in 15A NCAC
18A.1961 Table V(a),upon written request by the professional engineer and written approval by the owner of the wastewater
system.
Brian if you can type an email to Joe and me waiving the owner and LSS being present at the post construction meeting
and then Joe reply and acknowledge if you approve. Once we have that I can sign off on the ATO.
