in our October 13, 2010 inspection letter have been corrected for the referenced project. We have notified Acute and Home Care Licensure and Certification Section that the project is
approved for use as of October 20, 2010. As confirmed by email from Mr. Scott Echelberger on October 20, 2010, upon opening OR J you will immediately take existing Room 4 out of service
by disconnecting and severing the medical gas lines that feed that specific operating room and transmit pictures of the disconnection as documentation. Please use our Project No. HL-7918-TF/MS/ALH
