Laurel Dr. Maiden, South Carolina Dear Mr. Lane: At your request, the undersigned performed a site visit for observation and testing and calibration of the completed installation of the on -site wastewater septic system constructed at the referenced address. The system was to be constructed to plans and specifications prepared by Unifour Engineering & Testing Laboratories, PC (Unifour) dated 22 July, 2010 and approved for construction under permitting issued by the Catawba County Health Department. Revisions to plans and specifications requested to and approved by the Engineer, and authorized by Catawba County Health Department representatives, were as follows: 1. Re- location of the planned septic and pump tank as requested by Contractor (see sketch attached) 2. Revision of pumped effluent distribution to omit LPP valve box and line distribution, replaced with pressure manifold sizing design for distribution to gravity flow lines installed per T &J Panel manufacturer's requirements. (see attached PM sizing calculation sheet) The undersigned was able to directly observe the constructed pumping equipment setup, manufactured pressure manifold and installed T &J panels. Pumping operations were confirmed with supply valve settings adequate to maintain design manifold head at 2 feet as observed via temporary standpipe at the manifold, and appropriate float switch elevations were set for pump run time and alarm operations. All T &J panels appeared to be installed on grade lines as required and in accordance with manufacturer's installation requirements. Our observations and testing indicate that the septic system, as constructed, is in general accordance and complete with respect to project plans and specifications.
