Reproduction of Commercial Plan Request

I agree to pay the archive fee to Catawba County at the time of request. I understand
that is my responsibility to pay Accent Imaging any fees associated with the
reproduction of the above listed prints at the time of pick up. I agree to pick up the
reproduced prints at Accent Imaging located at 1630 Tate Blvd SE., Hickory, NC,
(Phone # 828-322-5050) within 48 hours of notification that prints are ready. I can be
contacted at the following phone number when plans are ready (E21D D.44-01'7 .
A 24 hour period will be required to locate and deliver requested plans to vendor for

reproduction. The time required for reproduction will be determined by the vendor and 1
will be the responsibility of the customer to coordinate acceptable completion./
