I put a note up on orange paper on the sign in/out sheet on the classroom door and on the refrigerator so the parents/staff would see the reminder about labeling/dating bottles every day. 11/21/17 After Prayer Breakfast when f got to the center, Diane told me that she had spoken with Ellie's Mom and she would not be back to the center.Jena was very upset and concerned that her child had drank another mother's breast milk. Diane let her know that we understood her concerns and would find out what was in the bottle and get back to her today. After further investigation the staff figured out that they gave Elyse Touron's bottle to Ellie Wallace. When I asked how this happened I was told by Maria that Ana gave the bottle to Ellie.There was no label with the child's name on it or date which is a State of NC requirement. I had both teachers come to my office separately. Maria Brincovan told me that she doesn't know who took the bottle out of the refrigerator but Ana did . feed it to the child. When they realized what had happened they told Diane who told me. I explained to Maria how serious this was as Elyse's is a breast fed baby and we do not know what was in the bottle. Also about the state rule and how we had spoken about it a few weeks ago when the Infant teachers told me that some parents were still not labeling/dating their child's bottles.At that time I told both Infant room teachers to check the bottles as they are brought in, and to speak with the parent about it or to tell me and I would contact them. Maria is to document everything and leave it with me before she leaves today. Maria told me that Ana was tired on the phone with her doctor, had a lot going on. Ana Serrano I asked Ana what happened and she told me she did feed the baby the bottle without looking at it. She said it was a very busy day in the room and she is human and made a mistake.Went over with her the correct procedure and she is documenting everything before she leaves today. I spoke with Rick Touron and asked him what was in the bottle. He told me it was formula. I thanked him and reminded him to label and date his bottles every day.
