No grading or construction activity is allowed in areas designated for system and repair without approval of Health Department.
There is an existing deck over the first drainfield trench. From what is visable, the deck does not have footers. It is supported on concrete blocks set on
the ground surface. It is not recommended [hat anything cover the drainfield. Absolutely no footers are allowed within 5 ft of the septic system.
Additionally. "small inflatable pools" should not cover the septic system.
Installation of the repair system may require the "re-routing" of water lines, power lines and/or plumbing lines.
'Permit Conditions
The issuance of this permit by the Health Department in no way guarantees the issuance of other permits. The permit holder
is responsible for checking with appropriate governing bodies in meeting their requirements.
This permit is being issued to re-designate repair area.
This permit is not intended for septic system installation.
Septic repair will be shallow placed 25% Reduction (pump required). 8.10 inches of fill for cover.
Final grade of repair system must divert surface water away from system. Additional conditions regarding the repair system will be cited on the AC
permit (when issued).
A 10x23 storage building (attached to the existing building) is proposed. 11 wil not impact the existing system or repair.
It is proposed to add a half bath to the existing accessory building. A full bath is NOT permitted. A half bath will not increase flow to the existing septic
system. This permit does not imply approval for the half bath. Approval for the bathroom will come from other agencies (buiding permits, zoning, andlot
plumbing inspections required - see the permit center for additional information).
Do NOT drive over the existing septic system or the repair area. Gravel driveway must not encroach on existing drainfield.
The Improvement Permit shall be valid for 6 years from date of issue with a site plan (means a drawing not necessarily drawn to
Site ® Ian scale that shows the existing and proposed property tines with dimensions, the location of the facility and appurtenances, the
site for the proposed Wastewater system, and the location of water supplies and surface waters).
Plat The knprovement Permit shall be valid without expiration with plat (means a property surveyed prepared by a registered land
surveyor, drawn to a scale alone inch equals no more than so feel, that includes: the specific location of the proposed facility
and appurtenances, the site for the proposed Wastewater system, and the location of water supplies and surface waters. Plat

The Department and Local Health Department may Impose conditions on the issuance and may revoke the permits for failure of
the system to satisfy the conditions, the rules, or this article This permit is subject to revocation if the site plan, plat, or Intended
use changes (NCGS 130A-335(f)). The person owning or controlling the system shall be responsible for assuring compliance
with the laws, rules, and permit conditions regarding system location, Installation, operation, maintenance, monitoring,
reporting, and repair (.1938(b)}
