'Site Modifications
No grading or construction activity is allowed in areas designated for system and repair without approval of Health Department.
Initial septic area and septic repair area must be fenced off and protected during house construction. Absolutely NO driving, grading, cutting, or filling is
permitted within either septic area.
'Permit Conditions
The issuance of this permit by the Health Department in no way guarantees the issuance of other permits. The permit holder
is responsible for checking with appropriate governing bodies in meeting their requirements.

submitted by Piedmont Design.
Septic installer must follow engineered plans EXACTLY. Plans specifiy pre-construction site preparation, installation instructions, and final landscaping.
Owner is responsible for continued "up-keep" of septic repair area. See engineered plans for details.

This Authorization for Wastewater System Construction shall be valid for a person equal to the period of validity of the Improvement Permit, not
to exceed five years, and may be issued at the same time the Improvem ent Permit issued (NCGS 130A-336(b)). If the installation has not been
completed during the period of validity of the Construction Permit, the Information submitted in the application for a permit or Construction
Authorization Is found to have been incorrect, falsified or changed, or the site is altered, the permit or Construction Authorization shall become
invalid, an j may be suspended or revoked (.1937(g)). The person owning or controlling the system shall be responsible for assuring compliance
with the laws, rules, and permit conditions regarding system location, installation, operation, maintenance, monitoring, reporting and repair
(1938(b)).

Additional Permit Conditions for the Improvement Permit
And Authorization to Construct the Wastewater System

1. This system is designed to serve a 2-bedroom single family residence only with
a daily design flow of 240 gallons per day. All construction and system
component installation shall be in accordance with plans approved by NCDENR,
DEH on January 26, 2010 prepared by Piedmont Design Associates, PA, dated
December 28, 2009, and referenced as attached.
2. The septic system contractor shall be required to lay out the entire system and
schedule a pre-construction conference with a representative of Catawba County
Environmental Health on site prior to initiating construction of the system.
Installation Contractor shall be certified at a minimum Level 4.
3. All prefabricated concrete pump tanks shall be leak tested and approved prior to
installation of pump system components. Pre-approved and specified plastic tanks
are exempt from this requirement.
4. All components of the system shall be installed in accordance with design
engineer and manufacturer's specifications and shall be demonstrated as properly
functioning prior to approval.
5. The design engineer shall be present at construction as sufficient to verify that the
installation has been completed in accordance with all plans and design
specifications.
6. A complete set of "as built" diagrams, certified by the design engineer, shall be
submitted to Catawba County Environmental Health upon completion of
installation and prior to final approvals.
7. Final landscaping of the nitrification fields shall be performed by placement of
approved soil cover over the entire field, slightly mounded in the center, and
gently sloped to the sides to shed surface water from the field. Seed and straw
shall be applied to the finished surface and appropriately maintained.
8. Pressure head at headworks shall be maintained as per drip system manufacturer's
specifications.
9. The system is categorized as a Type V. A contract with a certified subsurface
system operator, signed by all required parties shall be submitted to Catawba
County Environmental Health upon completion of installation and prior to final
approvals. Required ORC inspection frequency is twice yearly.
