SEE ATTACHED CONSTRUCTION AUTHORIZATION FOR SITE PLAN AND PERMIT CONDITIONS Pan The improvement Permit shall be valid for 5 years from date of Issue with a site plan (means a drawing not necessarily drawn to Site, scale that shows the existing and proposed property lines with dimensions, the location of the facility and appurtenances, the site for the proposed wastewater system, and the location of water supplies and surface waters). Plat The improvement Permit shall be valid without expiration with plat (means a property surveyed prepared by a registered land surveyor, drawn to a scale of one inch equals no morethan 60 feet, that includes: the specific location of the proposed facility and appurtenances, the site for the proposed Wastewater system, and the location of water supplies and surface waters. Plat also means, for subdivision lots approved by the local planning authority and recorded with the county register of deeds, a copy of the recorded subdivisions plat that is accompanied by a site plan that is drawn to scale). The Department and Local Health Department may impose conditions on the issuance and may revoke the permits for failure of the system to satisfy the conditions, the rules, or this article This permit is subject to revocation if the site plan, plat, or intended use changes (NCGS 130A-335(1)). The issuance of this septic system permit shall not, at any time, imply permission to occupy this accessory structure as a habitable dwelling in violation of Catawba County Zoning requirements. 2. The existence of a full bath in the structure, as interpreted by NCDENR officials, requires a minimum 240 gallon per day design flow, as it constitutes a dwelling unit for design flow purposes only, in accordance with 15A NCAC 18A.1900 (Laws and Rules for Sewage Treatment and Disposal Systems), Rule .1949a. 1,7 C Page 4 TRACKING INFORMATION Date Calls 09/02/09 1" Contact - Discussion Only WLS2004-00500 Site Ready to be Flagged Site Flagged Site Ready to be Evaluated Site Evaluated Approved for Issuance Other Date Comments/Field Notes 09/02/09 Met on lot at request of Ed Rivers to survey lot for potential expansion of septic system due to concerns over additional building on lot. Current system is a 4BR pump system which has 1000 gallon septic and pump tanks and (6)58' lines of 25% reduction drain field according to original permit issued by George Pendergrass. Undetermined as to type of distribution to system but there was not a pressure manifold visible near drain field, so it may be a possible d box. Measured to determine available space for expansion and there appears to be an area of approximately 70' depth by 60' width to install additional drain field and allow for repair. In accordance with 15A NCAC 18A.1952 , it appears that any additional flow to system will require a larger tank. Minimum tank size would likely require a minimum 1200 gallon tank based on V=2Q where V is liquid capacity of tank and Q is daily design flow. 1200=600(2). An alternative to removal of existing tank and installation of larger tank would be to install a separate 1000 gallon tank in series with the existing tank or hooking structure up to a separate system. Evaluated an auger boring below existing system and it appears that soil has a .35 LTAR with SCL to a depth of at least 36". Soil appeared to be well drained with no signs of mottles along with being very friable, slightly sticky and slightly plastic. At 600gpd flow, an LTAR of .35 and utilizing a 25 % reduction system, a system would require a minimum linear footage of 428'( 600/.35/3*.75). The system currently has 348 linear feet of drain field requiring an additional 80 linear feet of drain field.
