Permit Conditions: All parts of septic system and any future repair system must be minimum: 50' from any well, 10'
from property lines, 25' from house( up slope),50' from lake. Lines to be installed on contour. Do not
grade,drive, or fill over system or repair area or permit will be revoked. Lot is repair exempt and
recorded deed and map are attached with plan case. Verified with mappers for GIS that lot lines
have not changed since recorded in 1963 on 6/10. Due to location of home and lake setback, a
grinder pump must be used. A 1000 gallon settling tank must be placed in series with a 1000 gallon
septic tank with tee and filter then gravity feed to 5 40' 50°!o reduction trenches with a distribution
box. Prior to issuing any Authorization to Construct or well permit, a preconstruction conference
with building contractor and septic installer will be required. System will be shot in and all
components of system, home, and well setbacks for this lot and adjacent lots will be required to be
put on a survey site plan prior to issuance of AC.

approve the initial system installation, or the suspension/revocation of existing permits.
The issuance of this permit by the Health Department does not guarantee the issuance of other permits. It is the responsibility of the
applicant/property owner to insure that all Catawba County Planning/Zoning and Building Inspections requirements are met. This
Improvement Permit is subject to revocation if the site plan,plat or the intended use changes,or if site conditions are altered. The
Improvement Permit is not affected by a change in ownership of the property. This permit was issued in compliance with the
provisions of the North Carolina 'Laws and Rules for Sewa.e Treatment and Disposal Systems' (15A NCAC 18A.1900). Neither
Catawba County nor the Environmental Health Specialist warrants that the septic tank system will continue to function
satisfactorily for any given period of time.
