Steven Price 06/03/2016 AUTHORIZED STATE AGENT APPROVAL DATE Permit Expiration Date: 06/03/2021 No grading or construction activity is allowed in areas designated for system and repair without approval of the Health Department. ehpermit 06/06/2016 13:56 Page 2 of 4 EHPR 04-2016-23695 3375 Bethany Church Rd, Claremont • Do not cut,drive, fill, or grade over septic or repair areas. • Septic system must be 10 ft from property lines; 50 ft from wells and streams; 15 ft from a basement; 15ft from ditch; and out of right-of-ways and easements. • Install a new 1,000 gallon septic tank unless the existing septic tank can be shown to be structurally sound, in good working condition, and can be fitted with a sanitary tee and filter. • Install 300 linear feet of 25%reduction system; (3) 100 ft lines on contour with a maximum trench depth of 26 inches. The lines will need to be started high on the landscape above pit 1 and away from pit 2. • If the installer has questions, they should contact the Health Department and schedule a site meeting. Due to limited installation depth for the lines a pump may be required if proper fall from septic tank cannot be achieved. • Final grade of septic area must shed surface water off and away from system.
