Good deal. Just wanted to know what was going on. But we also wanted to make sure about the address on the check
itself and him cashing it before we mailed it to him. The address on the check is the property address he was working on
and not his business address. Sindie would this be a problem him cashing? Does it need to have his business address?
If not we will mail it, but if it is then a new check would need to be cut. Just wanted to make sure.

Hi Scott-
The correct address for Mr. Mauldin has been obtained. Mr. Mauldin contacted the Manager's Office before Mike
retired and requested a refund because the well in question didn't need to be abandoned. NC DOT had provided the
incorrect information. We issued the abandonment permit but no work had been done. I discussed w/Mike and w/
Mary and made the decision to issue the refund. I regret any confusion in this situation. Please let me know if you have
any questions and I'd be glad to discuss w/you.

Hey and good morning folks,just wanted to comment a few things about this well abandonment refund for Mr. Greg
Maulden:
• The address on the check is the physical address of the property in question and not the guy's business who was
going to do the work. Will this pose a problem? Since we were not involved with the refund in question,the
wrong(possible) address is on it. Did not want to mail the check to him and he not being able to cash it. He had
been a problem with this case as well as 4 others(well abandonments) so did not want to upset him more than
he has been with our process if he could not cash the check.
• Please next time let us know the situation where a refund has been given to an individual so we know how we
need to process. Did Mike know this before he left by chance and just forgot to tell us? Since a well
abandonment approval was issued,we would not want him to call us down the road and say it is abandoned and
then have us go out there when a refund was given and the approval be voided.

Would you have any information on this refund. The check came back "return to sender no such
number unable to forward". We did not know that the case for the transaction number that was
referenced had been refunded so permit has not been voided and refund has not been done in
energov.
