Owner must contact health department 6 months prior to expiration for permit renewal.
System Installation Comments: Installed 315 linear feet of infiltrator at 36"depth based on a .375 LTAR to help meet
multiple 100'well setbacks and allow additional room to drill new proposed well on lot.
Well not drilled at time of inspection. See surveyed site plan. Adrian Pruett LSS had
shot in and flagged additional drain field during proposal and proposed a .4 LTAR.
System was designed at lowest range of LIAR but determined during preconstruction
conference with installer that total amount of drain field could be lowered due to
installation depth and saprolite conditions. Lower line marked in field not installed. Line
lengths were 70',62' 58', 54'46' 25'. End of trenches were to be marked with blue flags
provided by EHS to installer after system was covered up.
PERMIT CONDITIONS:
1. All maintenance, monitoring, and performance requirements shall be in accordance with
15A NCAC 18.1900, Rule .1961
2. Operation & Maintenance Specifics:
Subsurface system operator required? Yes No_X_
If yes, see attached sheet for additional operation conditions, maintenance and reporting.
This system has been installed in compliance with applicable North Carolina General Statutes, Rules for Sewage
Treatment and Disposal, and All conditions of the Improvement Permit and Construction Authorization.
